This track will explore various aspects of the data and analytics ecosystem that power success in value based care, including case studies from high-performing entities who have wrangled data to drive them forward.

The transition to value based health care is long past its debut, but not yet at full maturity--many value based models and experiences remain in the early stages. This track will explore the tried and true practices that have been shown to reduce cost, improve quality, and center the patient experience within value based care.

The focus on health-related social needs has hit an all-time high, but where does the focus on screening for unmet social needs and connecting patients to appropriate social care providers support or advance the goals of value based care? This track will explore these questions and provide case studies of how providers and payers are blending these two important policy goals.

Much has been said and promised about integrating social care support services, such as housing, nutrition, transportation, into the health care delivery system. Many states, including New York, are embarking on a large-scale approach to test how integrated the two sectors can become. This track will explore how several unique organizations in New York plan to make this vision a reality.

Value based care models have often started with the primary care provider in mind, with some advanced, mature models including specialty care. But what about long-term care providers, who care for hundreds of thousands of New Yorkers? This track will explore some of the emerging, innovative models for taking risk for this specialized population.
